Change Can be Very Good

Even small changes are good, they are like mere drops of water tumbling onto the water, creating ripples of better things to come. Life is always an evolution and you should be the one to start that next ripple of events on your pathway to the good life. As we begin to assist you on your journey, we need to work together to develop a plan of action, decide on appropriate choices before a crisis, implement preventative measures and finally give yourself "peace of mind", a gift you and your family deserve.

Starting the Process is Easy

Your relationship with GoodLife Pathways and one of our consultants starts with The General Overview Form and provides a general assessment or overview of your situation. It is the beginning of our work to assess, plan and help you implement a sensible solution for your specific needs and goals.

Completing and sending the General Overview Form does not put you under any obligation to work with any of our service providers. There is a $250 fee for the interpretation of your General Overview Form and a meeting with the consultant (Bronze Service Level 1) who will make determinations and give you recommendations.

GoodLife Pathways is committed to honoring the privacy of visitors to our website. We do not share, sell or exchange any information which may be submitted by you. Only with your written approval, do we exchange or share the information submitted by you to a provider of your choice who will be assisting in the process.

 Three Other Assessments

Three other forms are needed for Silver Service Level 2 and Gold Service Level 3 in order to complete your profile. These additional forms will be provided to you at your first meeting with the consultant. They include:

  • The Health Needs Assessment:  Completing this assessment will also help your doctor, case manager or other health care professionals determine the best direction or treatment for your health issues. this is information is not shared with anyone without HIPAA privacy rules being considered.
     
  • Transition Housing Evaluation: What would be the best answer to your housing needs? Today we have large and small residence communities for seniors, where the building size, design and location provide lifestyle choices for aging adults. Adult children of Seniors must take into consideration safety and health issues for their parents in addition to the desires of their parents. There is a better way and wrong way of encouraging a parent to make these choices at the right time. These decisions can affect the entire family, the dynamics between the family relationships and the happiness of the parent for years to come. 

  • Monthly Expense Worksheet: This is not a financial plan and we do not ask for all of your private financial information. The worksheet provides you an understanding of how much it will cost for your transition and subsequent monthly expenses at your new residence community.
